Full Time MDS Coordinator
Salary Commensurate on Experience
Position Summary
The MDS Coordinator is responsible for assuring the timely completion of accurate resident assessments and interdisciplinary care plans that meet Federal and State guidelines. This includes identifying resident acuity and needs, helping to determine specific care needs and communicating needs and expectations to families and responsible parties. 
Essential Duties and Responsibilities
  • Coordinates assessments and interdisciplinary care plan according to regulatory requirements in timely fashion
  • Identify resident acuity and needs, help determine specific care needs
  • Effectively communicate needs and expectations to families and responsible parties
  • Ensure accurate and timely completion of the MDS/CAAs/Triggers sheet for residents
  • Initiates care plans and supporting activities that will result in best possible outcome for assigned residents
  • Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribes MDS timeframe
  • Must ensure important resources are made available to patients and that patient care is delivered effectively and to a satisfactory standard
  • Responsible for managing Medicare and Medicaid assessments
  • Must be willing to assist other nursing staff in daily shift duties
  • Other duties as assigned by the Director of Nursing or Nursing Home Administrator
Knowledge, Skills, and Abilities
  • Working knowledge of nursing services, rehabilitation, therapy, and resident care
  • Possess skills to continuously comprehend and communicate with residents and co-workers regarding written plans of care, and to respond to emergency alarms
  • Ability to remember multiple messages, information, and task assignments given to self and others
  • Ability to communicate and collaborate with others and work as a team
  • Strong interpersonal skills
  • Ability to demonstrate strong leadership, communication, education and collaboration and counseling skills
  • Ability to communicate proficiently through technology
  • Ability to identify and implement appropriate patient communication strategies and overcome accessibility barriers, as required
  • Ability to demonstrate continual learning skills, effects changes in approach to care based on established, evidence-based practice
  • Ability to determine appropriate course of action in more complex situations
  • Ability to work independently, exercise creativity, be attentive to detail and maintain a positive attitude
  • Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work
  • Ability to maintain confidentiality of all medical, financial, and legal information
  • Ability to complete work assignments accurately and in a timely manner
Required Qualifications
  • Must be a licensed nurse in the state of Illinois, preferably a RN.
  • Prior MDS/Care Plan experience required.
  • Prior experience in a long-term care facility is required
  • RAC-CT designation preferred.
Physical Requirements
  • Occasionally transfers up to 75 pounds with assist while lifting and positioning and transferring residents
  • Pushes/pulls medication carts requiring force of 10-30 pounds
  • Pushes/pulls residents weighing 75-300 pounds in wheelchairs
  • Reposition residents weighing 75-300 pounds in bed for treatments and nursing procedures as needed
  • Stands and walks up to 75% of shift to provide resident assessments
  • Stoops, bends, and squats as required to reach lower drawers on medication carts, adjust resident’s items, etc.
  • Able to wear gloves and other PPE
  • Identifies residents, call lights, and unusual occurrences on unit at a distance of 50 feet
