Clinical Care Coord / RN - $5,000 sign on bonus

RN's with acute care experience are eligible for a $5,000.00 sign on bonus paid out in four installments with a work commitment. Candidate must obtain ONS Chemo Biotherapy competency within one year of hire.

We invite you to become part of a comprehensive collaborative care team dedicated to customer service and clinical excellence.  Northern Light Health Mercy Cancer Care and Infusion Program Located on the Fore River Campus is seeking a 36-hour experienced nurse to join our Comprehensive Collaborative Cancer Team. The RN Care Coordinator is responsible for day to day navigation of patient care in a supportive environment from initial contact through follow-up visits, and treatment.
The Clinical Nurse Coordinator is a member of the multidisciplinary team and supports a smooth transition of patients from diagnosis and active treatment into survivorship, chronic cancer management, or end-of-life care. The Clinical Nurse Coordinator demonstrates leadership in coordinating comprehensive and continuous patient-centered care through active collaboration, communication, and coordination of internal and community-based resources and services. Works across health care disciplines to assist in coordinating patient tests, procedures, appointments, and treatments, and assist patients in overcoming barriers to care.
Summary:
The Clinical Nurse Coordinator is a member of the multidisciplinary team and supports a smooth transition of patients from diagnosis and active treatment into survivorship, chronic cancer management, or end-of-life care. The Clinical Nurse Coordinator demonstrates leadership in coordinating comprehensive and continuous patient-centered care through active collaboration, communication, and coordination of internal and community-based resources and services. Works across health care disciplines to assist in coordinating patient tests, procedures, appointments, and treatments, and assist patients in overcoming barriers to care. He/she provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The Nurse Coordinator is responsible for routine nursing aspects of the Cancer Program, which include clinical care, telephone triage, medication adherence, symptom management, management of patients on oral chemotherapy, data gathering and chart updating, patient education, and monitoring and follow-up of patients. The Clinical Nurse Coordinator collaborates with an interdisciplinary team to ensure high quality care across all delivery sites and serves as a single point of contact for patients, caregivers, referring physicians and the community.
The Clinical Nurse Coordinator works closely with physicians to develop data processes/analysis to track patient progress and outcomes. The Nurse Coordinator is responsible for participation in and compliance with all patient safety initiatives and regulations associated to the position. Conducts all job responsibilities according to the Mission and Values of Mercy Hospital. Is fully compliant with HIPAA regulations.
Other information: BLS Required.
Credentials: Maine Registered Nurse License or eligible to obtain
Education: BSN preferred.
Experience: Minimum four (4) years clinical experience as a registered Nurse in an acute care setting is preferred. Experience in critical care, oncology, care management is desired. Excellent interpersonal/communication skills are required as the incumbent interacts with patients, families, physicians, and other caregivers throughout the continuum. Must have excellent analytical and problem-solving skills to assess patient care needs. Must have the judgment and initiative to operate independently and autonomously in the assessment, development, implementation and follow up of care needs with minimal guidance from the Practice Manager. Must be highly accountable for timely and accurate planning and preparation to assure safe and cost-effective utilization of available healthcare resources in the coordination of patient care.
