Public Health Nurse I/II

The Vermont Department of Health has a dynamic opportunity for an enthusiastic and experienced nurse who wants to join a dedicated interdisciplinary team that will make differences in the health of communities in the Northeast Kingdom. We are seeking a well-organized and energetic nurse with great communication skills to complete our team of public health professionals in the Newport District Office. 
We seek an enthusiastic and dedicated nurse to become our new Maternal and Child Health (MCH) Coordinator and School Liaison. You will work at the systems level, encouraging coordination of services for pregnant women, infants and children. This important work is achieved by developing and maintaining relationships with the local systems of care for children and families and facilitating systems change to improve MCH outcomes. There is a prevention focus, creating a foundation for health and wellness from preconception through early childhood.
 
Major duties include:
1.   Develop and maintain relationships to local system of care for children and families with the goal of assessing community needs, sharing best practices, and facilitating systems change to improve MCH outcomes
2.   Identify health disparities and service gaps related to the MCH population through regular review of local, state and national data to maintain awareness of trends in the MCH population and share data with community partners
3.   Through participation with MCH related programs and partnerships, inform and contribute to the design of systems that serve special populations, including pregnant women/infants with opioid addiction, children with special health needs, children in custody and foster care, victims of domestic violence, and families living in poverty.
4.   Support School Nurses across the region and support school health initiatives including infection control practices.
 
As a public health nurse, you also work with a public health team on emergency preparedness, epidemiological investigation, immunizations, and substance abuse prevention. Duties are performed under supervision of Public Health Nurse Supervisor.
