Nurse Practitioner Lightning Rounder

The hospital is comprised of 545 beds and is a Level I Trauma Center, an Emergency Heart Care Station and a 911 Receiving Hospital. Job Description:

Under general supervision, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care.

Duties & Responsibilities:

1. Initiates readiness for patients discharge by rounding all constraint areas to plan discharge, and patient admission

2. The Lighting Rounder (LR) Identifies bottle necks and barriers trends in the effective discharge and admitting process, and reports to the Capacity Management Expediter and/or Director of Capacity Management.

3. The LR identifies bottle necks, delays and barriers for an effective, safe and opportune patient discharge, and patient admissions while correcting them within his/her/their professional scope of practice.

4. Escalates unattended or delayed patient care needs that are identified under their responsibilities

5. The Lighting Rounder implements patient care when needed.

6. The Lighting Rounder collaborates with the RN to identify expected outcomes for a planned or expected safe discharge of a patient

7. The Lighting Rounder assists in the assessment, planning, intervention, and evaluation of nursing care during the light rounds.

8. Provides ongoing assistance to the interdisciplinary patient care team to accomplish the departmental goals and metrics

9. The LR, when required or when the need arises, consults with medical, nursing and other staff involved in treatment to clarify issues and secure proper documentation of planned, scheduled and potential discharges or admissions.

10. Removes barriers to support a holistic environment in coordination with patient care team, and under the Director of Capacity Management and/or designee

11. The LR utilizes advanced skills in patient care.

12. The LR collects comprehensive data pertinent to the patient’s health and/or situation and utilizes critical thinking skills within the scope of practice.

13. The LR contributes to develop and executes a plan that prescribes strategies and alternatives to attain expected outcomes.

14. Utilizes NP functions to optimize department outcomes within the Lighting Rounder’s scope of practice.

15. When required, the Lighting Rounder consults with medical, nursing and other staff involved in treatment to clarify issues and secure safe, effective, and efficient discharge and admission while securing documentation of the records

16. Provides ongoing technical assistance to the interdisciplinary patient care team.

17. Initiates action and/or works with Social Service Department to monitor timely discharge planning.

18. Collaborates with the RN, physician and healthcare providers to promote the well-being of patient/family and supports patient/family preferences.

19. Collaborates with RN, physician and healthcare providers to deliver education which includes information related to patient specific needs.

20. Demonstrates awareness and sensitivity to the rights, dignity, and confidentiality of patients/significant others.

21. Seeks knowledge and competency about current and new practices.

22. Keep themselves informed of changes in regulations, procedures and treatment standards prescribed by the hospital, regulatory and/or reimbursement agencies.

23. Incorporates evidence-based practice into daily activities to ensure practice is based on current research or data.

24. Assumes accountability and responsibility for self-direction.

25. Integrates hospital policies and procedures into practice.

26. The LR communicates effectively in a variety in all areas of practice and utilizes communication skills to properly notify the RN of changes in the patient condition.

27. Provide straining and orientation to less experience staff.

28. Anticipates and communicates patient care needs to other disciplines and follows through to resolution within their scope of practice.

29. Follows policies and procedures (infection control, absences, safety, use of cell phones, computers and hospital equipment, etc).

30. Respects and protects patient privacy and confidential information in all settings.

31. Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.

32. Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions and implements therapeutic and corrective measures.

33. Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician’s intervention.

34. Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.

35. Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.

36. Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.

37. Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment and other pertinent information. Identifies health and medical risks and needs.

38. Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.

39. Orders, conducts, and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.

40. Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources.

41. Provides education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.

42. Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention and health promotion, records and maintains a system of relevant documentation.

 

Minimum Qualifications:

• Current New York State professional nurse registration

• Current New York State nurse practitioner registration, with prescription privileges.

• Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

• Certification by as national certifying body in her/his specialty area.

• Credentialed by Medical Board.

• Masters or Doctorate Degree
