Community Health Worker- Nurse Family Partnership (NFP)- Marion County

This position will help support the Central Indiana region so ideal candidates will live in our around the Marion County/Indianapolis area. The salary range for this position is $21-$26 per hour and includes full benefits (below):

 

Nurse-Family Partnership (NFP), a maternal and early childhood home visiting program, fosters long-term success for first-time parents, their babies, and seeks to address some of the serious systemic problems affecting our community. Building on this research, the NFP Community Health Worker addresses the fragmented structure of the social services industry by taking a holistic, whole-family approach.  The NFP Community Health Worker provides expert advice and troubleshooting solutions to assist NFP Nurses in directing their participating families to quality resources in the community based upon each family's specific needs, strengths, and motivation, as identified by the Nurse Home Visitor.  Services might address immediate needs or short-term problems.  The primary focus is to create long-term solutions, accountability, growth, and development of human potential consistent with the Nurse-Family Partnership model.   

 

The variety of services necessary to address community needs is greater than any one organization can manage. The NFP Community Health Worker’s effective application of these wrap-around services is achieved, in part, through existing and new partnerships with community organizations and resources capable of providing services and opportunities that best fit each individual’s context, need, and motivation.

 

Example Duties and Activities 

  • Builds solid relationships with NFP staff, NFP families, and maintains networks with key community partners including Goodwill staff (community agencies, educational institutions, financial liaisons, and employers) to enhance the menu of wrap-around services provided.  Serves as the primary point of contact with community partner organizations to receive and manage client referrals.
  • Creates easily accessible rapid-response and long-term solutions for NFP nurses and families to connect effectively to quality community resource networks.
  • Ensures timely and accurate entry of required information into NFP databases and files.  Generates status reports and makes adjustments as needed.  Tracks, documents, and communicates program-specific metrics utilizing multiple reporting systems.
  • Participates in NFP team meetings, case conferences, and one-on-one reflective discussions with NFP supervisors and nurses.
  • Helps NFP nurses achieve evidence-based outcomes.
  • Participates in and, at times, guides the development and implementation of continuous innovation projects that improve operational efficiency and evidence-based outcomes.

Technical Knowledge - Has working knowledge of G Suite and MS Office.  Prior database knowledge is an additional plus.

Other Requirements

Ability to occasionally work a flexible schedule beyond normal business hours.

Is willing to travel to participants’ homes, workplaces, community centers, and other venues at times that are convenient to individuals being served.

Background Screening - All candidates must pass a pre-employment background check.  Must have a valid driver’s license, up-to-date auto insurance, and a good driving record.
