
LPN
We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will possess a strong background in patient assessment and primary care, with experience in IV infusion and familiarity with medical terminology. This role requires excellent communication skills, the ability to work independently, and a commitment to delivering patient-centered care.

Duties
- Conduct thorough patient assessments to determine individual care needs and develop personalized care plans.
- Administer IV infusions and manage medication schedules as prescribed by physicians.
- Provide education and support to patients and their families regarding health management, including family planning and memory care.
- Collaborate with healthcare teams, including clinics and teaching hospitals, to ensure continuity of care.
- Utilize electronic health records systems such as Cerner to document patient information accurately.
- Assist with durable medical equipment (DME) setup and training for patients as needed.
- Monitor patient progress and adjust care plans accordingly, maintaining open communication with physicians and family members.
Requirements
- Valid nursing license in the state of practice.
- Proven experience in home health care or a related field is preferred.
- Strong knowledge of medical terminology and patient assessment techniques.
- Experience with IV infusion procedures is highly desirable.
- Familiarity with electronic health records systems, particularly Cerner, is a plus.
- Ability to work independently while managing multiple patient needs effectively.
- Excellent interpersonal skills for effective communication with patients, families, and healthcare professionals.
